actually i know 10Rs...right assessment, r.teaching, r.medication, r. dose, r. time, r.route, r.client, r.documentation, r. evaluation and right to refuse. that's the most recent i know.

personality, dose, route, medication, time, and today a lot of topical books state a sixth right to be their pain rating...location, type and number 1-10 of anguish.
